Apply in person, by phone with a Patient Financial Advocate, or by mailing a paper application. In Ohio and Nevada, uninsured patients at or under 400% may be approved automatically through third-party income screening.
Deadline
Within 240 days of the first billing statement, or from the date a collection agency returns the balance, whichever is later.

Who the program is for

The policy names uninsured patients as its principal beneficiaries. Insured patients can qualify only in limited circumstances, and Cleveland Clinic must be in network.

Income is not the only route. Patients whose bill for a single episode of care exceeds 25% of their annual family income may qualify at any income level, and so may patients facing terminal illness or another catastrophic condition.

In Ohio, uninsured patients are screened against third-party income data at scheduling or before admission, and anyone found to be at or under 400% of the guidelines is approved without submitting anything at all. Most patients are never told this happens.
